Perpetual Succor Hospital - Patient - Visitor Guidelines - Philhealth Benefits Availing Procedures

Submit to the Philhealth Section the following documents prior to discharge patients.

1. Duly accomplised Philhealth Claim Form 1(original)

2. Clear copy of Member Data Record or MDR

3. If MDR is not available, submit official receipt of applicable premium payment.

4. If qualified dependents is not listed in the MDR - submit applicable proof of dependency.

5. Agree with your attending physicians on how much is left to be paid for their services over the professional fee (PF) benefit.

6. After submission of all applicable documents, the Philhealth clerk will compute and deduct the benefits from the total hospital bill of the patient discharge.


(For Direct Filing / Reinbursement)

1. Duly accomplised Philhealth Claim Form 1 (original)

2. Philhealth Claim Form 2 (to be filled up by the hospital and attending physicians)

3. Official receipts of the hospital bill from the cashier.

4. Statement of accounts from the billing section.

5. Waiver from the hospital certifies patient no philhealth deduction of the hospital bill.

6. Operative record for surgical procedures performed.


AMENDING MEMBERSHIP DATA
Data amendment procedures

Submit duly accomplised Member Data Amendment Form or (M2) with the following supporting documents as applicable:


CORRECTION OF CHANGE OF NAME:

Original or Certified True Copy (CTC) of Birth Certificate, or in its absence, any two (2) of the following secondary documents:

1. Marriage Contract / Certficate, if Married

2. Passport

3. NBI Clearance

4. PRC ID

5. SSS / GSIS ID

6. Postal ID

7. Driver's License


CORRECTION OF DATE OF BIRTH

Original or Certified True Copy (CTC) of the Birth Certificate, or in its absence, any two (2) of the applicable supporting documents mentioned above.


CHANGE OF CIVIL STATUS

1. Single to Married - Original or CTC Marriage Contract / Certificate

2. Married to Widowed - Original or CTC of Death Certificate of Spouse

3. Marriage Annulled - Original of CTC of Annulment Papers


NEW / ADDITIONAL DEPENDENTS:

1. Legal Spouse - CTC of Marriage Contract / Certificate

2. Legitimate or illegitimate children - CTC of Birth Certificate

3. Stepchild/ren - CTC of MarriageContract / Certificate between the natural parent and the stepfather / mother and birth certificate of the dependent stepchild/ren

4. Disabled Child/ren 21 years old and above - CTC of Birth Certificate and Certification from the attending physician stating dependent is disabled

5. Adopted Child/ren - Court Decree of Adoption

6. Parents - CTC of Birth Certificate of member and any proof attesting the date of birth of the parents.
