Guidelines for Obtaining a Birth Certificate for Newborns

  • Form Collection: Obtain and complete the Birth Certificate Form from the Medical Records staff, located at the Ground Floor, open from Monday to Sunday, at 8:00am to 5:00 pm.
  • Form Submission: Return the filled-out form to the Medical Records section strictly within 24 hours or right after filling-out the form.
  • Verification and Printing: The Medical Records staff will verify the details, encode the information, and print the completed birth certificate.
  • Finalization and Approval: The birth certificate will undergo a final review and proofreading by the parents.  If all details are complete and correct, the parents shall affix their signatures to the form.
  • Receipt of Certified Copy: A certified true copy of the birth certificate will be released by the hospital to the parents for personal copy only.
  • Official Registration: The registered copy of the birth certificate will be available at the City Civil Registry Office-Manila City Hall  after One (1) month,  and Three to Six (3 to 6) months at the Philippine Statistics Authority (PSA) upon giving birth.
