For scheduled admissions, patients and their companions should proceed to the Admissions Section, conveniently located at the lobby of the Main Hospital building.

Prior to admission, our admissions team will require the following:

  1. Doctor’s order for admission, detailing specific instructions for the patient’s care
  2. Member’s ID issued by an accredited Health Maintenance Organization (HMO)
  3. Philhealth ID
  4. Persons with Disability or Senior Citizen ID, if applicable
  5. A government-issued ID
  6. Completed admissions forms
    • Admission and Discharge Record
    • Admission Contract
    • Patients Rights and Responsibilities form
    • Duties and Responsibilities of Patient’s Primary Representative

These documents are essential to ensure a smooth and efficient admission process.

  1. Pay the required deposit at the Cashier Sectionlocated at the lobby (not applicable for emergency cases, health cards, and corporate accounts)
  2. An admission kit and pillow will be provided by the admitting officer
  3. Patients will be endorsed, by the admitting clerk, to the OPD-ER for physical examination
  4. Once the physical examination is completed, a nurse on duty will accompany the patient to their selected room
  5. Companions may proceed to the patient’s room.

Discharge Procedures

To ensure a smooth discharge process, please follow these steps:

  1. Confirm with the nurse on duty when your attending physician has authorized your discharge.
  2. After receiving discharge approval from your attending physician, ensure completeness of submitted documents to Philhealth section for applicable Philhealth or HMO coverage.
  3. Complete the customer feedback form and submit to Billing section upon request of final hospital bill, or to the Cashier section upon payment.
  4. Obtain your summarized and itemized statement of account, and the attending physician’s professional fee from the Billing staff.
  5. Settle your Hospital bill and/or Professional fee to PSHM Cashier section only.
  6. After settling your hospital bill, go to the nurse station to have your room clearance signed by the nurse on duty.
  7. Collect all instructions for home medication, follow-up check-ups, and health advice from the nurse on duty or your attending physician.
  8. For Cesarean Section (CS) or Normal Spontaneous Delivery (NSD) patients: proceed to the Neonatal Intensive Care Unit (NICU) with your signed discharge pass to collect your newborn. Receive additional care instructions for your newborn from the pediatric nurse on duty and the attending pediatrician.
  9. Once all arrangements are complete, ask the nurse on duty or hospital operator for an orderly to assist with your luggage.
  10. Show your discharge pass to the guard on duty before leaving the hospital premises.
