Obtaining Medical Certificates and Other Medical Records
Request from Physician:
Obtain a written request from your Attending Physician and submit it to the Medical Records office, located on the ground floor. The office is open Monday to Sunday, from 8:00 AM to 5:00 PM.
Representative Authorization:
If a representative is requesting on behalf of the patient, an authorization letter from the patient is required, along with two (2) valid forms of ID and the written request from the attending physician.
Processing Time:
A certified true copy of operation records, discharge summaries, and laboratory results can typically be released within thirty minutes. Medical certificates are usually available within a day, and clinical abstracts can take 3 to 5 working days to process.
