Patients are entitled to a written notice of their rights and responsibilities before treatment begins.
If a patient is unable, their family or guardian may exercise these rights on their behalf.

Access to Care:
Patients have the right to care regardless of gender, age, disability, ethnicity, religion, or payment source, including the freedom to choose their physician.

Quality Care:
Patients are entitled to considerate, respectful care aligned with their values and beliefs, within PSHM’s mission, vision, and policies.

Informed Decisions:
Patients have the right to make informed decisions about their care, including understanding medical findings, treatment alternatives, and associated risks.

Research Participation:
Patients can consent or refuse participation in research studies or clinical trials, with full explanation provided prior to consent.

Advanced Directives:
Patients may create Advanced Medical Directives and designate a surrogate decision-maker.

Ethical Consultation:
Access to PSHM’s Ethical Committee for ethical concerns in medical care is available. Contact the Administration Office or, after hours, the Administrative Supervisor.

Privacy:
Patients have the right to privacy in care discussions, consultations, examinations, and treatments.

Personal Rights:
Patients can wear personal clothing, manage finances, send/receive mail, and associate freely, as medically appropriate.

Confidentiality:
All patient communications and records are confidential.

Communication:
Patients have the right to effective communication assistance and to be informed of any communication restrictions.

Care Planning:
Patients are entitled to participate in care planning and decision-making.

Pain Management:
Patients have the right to appropriate pain management.

Abuse Protection:
Patients have the right to be free from all forms of abuse and harassment.

Safe Environment:
Patients are entitled to a safe environment, including infection control and facility security.

Standard of Care:
Patients can expect health services to meet standard care levels and be informed of transfer risks and alternatives.

Care Alternatives:
Patients will be informed of realistic care alternatives when hospital care is no longer suitable.

Billing Information:
Patients have the right to an itemized bill and billing explanations.

Dispute Resolution:
Patients can access resources for resolving disputes, grievances, and conflicts.
