Benefits Usage Guide

Philhealth Requirements during Patient confinement:

Submit the following requirements to the Philhealth Section

For Employed (private or government)

1. Completed original PhilHealth Claim Signature Form(CSF) with employers signature.
2. Certification of Philhealth Premium Contribution from his/her employer
3. Proof of Contribution from members portal (Online PhilHealth portal)
4. Philhealth ID/any valid government ID
5. Updated original Member Data Record (MDR).
6. For undeclared dependents in MDR, submit proof of dependency birth certificate and or marriage contract
7. Fill out PhilHealth Member Registration  Form (PMRF) for proper updating

For self-employed/voluntary/individual paying member

1. Completed original Philhealth Claim Signature Form (CSF).
2. Official receipt of the latest premium payment (at least 6 months including the month of admission).
3. Proof of Contribution from members portal (Online PhilHealth portal)
4. Philhealth ID/any valid government ID
5. Updated original Member Data Record (MDR).
6. For undeclared dependents in MDR, submit proof of dependency birth certificate and or marriage contract
7. Fill out PhilHealth Member Registration Form (PMRF) for proper updating

For indigent/sponsored/group enrollment

1. Completed original PhilHealth Claim Signature Form.
2. Updated original Member Data Record (MDR) with validity period (current year of admission)
3. Proof of Contribution from members portal (Online PhilHealth portal)
4. PhilHealth ID/any valid government ID
5. For undeclared dependents in MDR, submit proof of dependency birth certificate and or marriage contract

For lifetime member/senior citizen (RA 10645)

1. Completed original Philhealth Claim Signature Form (CSF).
2. Updated original Member Data Record (MDR).
3. Philhealth ID/Senior Citizen ID.
4. Fill out PhilHealth Member Registration Form (PMRF) for proper updating

For Direct Filing/Reimbursement:

1. Completed original PhilHealth Claim Signature Form (CSF).
2. Completed original Philhealth Claim Form 1 (CF1)
3. PhilHealth Claim Form 2, filled out by the hospital and attending physicians.
4. Official hospital bill receipts from the cashier.
5. Billing section’s statement of accounts.
6. Hospital waiver certifying no PhilHealth deduction from the bill.
7. Operative record for any surgical procedures.
8. Laboratory results (if available)

Reminders:  No PhilHealth Requirements no deduction of PhilHealth Benefits.
